WIthdrawal Card Information
Withdrawal Card InfoMembers of Teamsters Local 445 who will be absent from work for more than a month, for any reason, are urged to get a withdrawal card. That way, they will remain in good standing and are not responsible for any monthly dues during their absence. A withdrawal card is issued to Teamster members that are not working for extended periods, such as, a leave of absence, lengthy medical leave, layoff, or terminated employment. When you are on withdrawal, you are considered to have an “inactive” status, and are therefore ineligible to exercise the rights of union membership, such as voting in elections or on contracts. It is your responsibility to obtain a Withdrawal Card in a timely manner, or you will be responsible for all the union fees that are due. Failure to request a Withdrawal Card may obligate you to pay back dues and a re-initiation fee. It is also your responsibility to inform your employer to restart your dues check–off when you return to work. When you return to work, you will not be charged any new initiation fees if you received a Withdrawal Card. The forms are available on the website, or you can get one from a Shop Steward or the Union Hall. The cost for withdrawal is 50 cents. Mail the form or visit the Union Hall at 15 Stone Castle Road, Rock Tavern NY 12575.
